What Surgeons Perform Lipedema Surgery
Lipedema surgery is not the same as cosmetic lipo, and it’s not something every plastic surgeon is trained to perform. In fact, most cosmetic surgeons avoid treating calves and ankles altogether due to the precision required and lymphatic risks.
Many doctors currently performing awake liposuction for cosmetic purposes come from various medical backgrounds, including dermatology, OB/GYN, primary care, and general surgery. Interestingly, plastic surgeons are often not trained in awake liposuction, as they typically perform all procedures under general anesthesia.
Regardless of a doctor’s specialty or training, no one is formally taught how to perform liposuction specifically for lipedema patients. That’s because treating lipedema requires a much more advanced and specialized approach than standard cosmetic liposuction. Lipedema liposuction is a specialty that requires self-training, advanced anatomical understanding, and experience treating large, complex fat deposits.

What Is Lymphatic Sparing Liposuction?
Lymphatic sparing liposuction is critical to successful lipedema treatment using micro-instruments, precise longitudinal cannula paths, and careful avoidance of deep tissue trauma to preserve the lymphatic system.
Unlike general anesthesia liposuction, which relies on large, aggressive cannulas, our method is gentle, safe, and strategically designed. We pair this technique with either SAL or WAL to make sure lymphatic integrity is protected.
Damaged lymphatics can lead to worsening edema and long-term complications. By preserving these pathways, we help control swelling and promote healthier, longer-lasting results.
Recovery
Understanding the stages of recovery is the best way to help you prepare physically and mentally for the weeks ahead. Here’s what to expect after your lipedema surgery:
- Surgery Day: You will be able to walk the same day as your surgery, but you should expect drainage, soreness, and swelling. Pain is usually manageable with medication, and compression garments need to go on immediately.
- Week 1: Early Healing: Drainage tapers off over the course of the first week. You’ll be mobile but tired, so you need to focus on getting the rest that your body needs. Bruising and swelling increase, so you shouldn’t be alarmed if you notice it. Light walking is encouraged, but again, rest is essential.
- Weeks 2–4: Peak Swelling: Swelling usually reaches its peak somewhere between the second and fourth week after your surgery. You may feel stiffer or more uncomfortable, but this is normal. Stay consistent with compression and gentle movement to help aid in your recovery.
- Weeks 5–6: Turning Point: Swelling begins to subside around the fifth or sixth week. You’ll likely feel stronger and more mobile. Most patients return to work and light activity, but heavy exercise is still off-limits.
- Months 2–3: Visible Progress: Inflammation decreases steadily over the second and third months, and your skin begins to retract. You’ll start to see the shape and volume change, though the results are not yet final.
- Months 4–6: Long-Term Refinement: Residual swelling resolves, and final contour improvements continue. By month six, many patients see the majority of their results, though changes can still evolve up to a year post-op.
